Do you accept my insurance?

SB Physio is moving toward a model that relies less on insurance so we can offer faster access, fewer restrictions, and more personalized care.

We are still contracted with several major health insurance companies, including Blue Shield, Cigna, and others.
Our last day in-network with Anthem Blue Cross is February 5, 2026.

We verify your benefits and bill your insurance when applicable.
As of July 1, 2025, SB Physio will not be enrolled with Medicare, and Medicare cannot be billed or reimbursed for services received here.

Is physical therapy covered by my insurance?

Coverage varies by plan and whether SB Physio is considered in-network or out-of-network for you.

In-network care (e.g., Blue Shield, Cigna): your plan sets co-pays, visit limits, referral requirements, and pre-authorization rules.

Out-of-network care (e.g., United Health Care, Aetna, HealthNet, Anthem Blue Cross after Feb 5, 2026): coverage depends on your plan’s out-of-network benefits and may offer more flexibility but different reimbursement rules. Many patients choose this path for faster scheduling and fewer visit restrictions.

For accurate details, contact your insurance using the member services number on your card and ask about:

  • In-network vs out-of-network benefits

  • Deductible status

  • Co-pay or co-insurance

  • Referral or pre-authorization requirements

  • Visit limits and maximum dollar coverage

What if I don’t have insurance coverage for physical therapy?

We offer straightforward private-pay options for patients without coverage or for services not billed to insurance. This pathway gives patients quicker scheduling, more flexibility, and access to the full scope of our movement and recovery approach.

For non-Medicare patients, we can provide a superbill for possible out-of-network reimbursement.

What do these insurance terms mean?

  • Deductible: The amount you must pay before your insurance plan contributes.

  • Co-insurance: A percentage of each visit’s cost you pay after your deductible is met.

  • Co-payment: A flat fee per visit, typically shown on your insurance ID card.
